Job Description:
A company is looking for a Care Navigator to support patients
enrolled in chronic care management and remote patient monitoring
programs.
Key Responsibilities
Conduct virtual consultations focusing on patient education related
to chronic diseases
Review and respond to remotely transmitted patient data such as
blood pressure and blood glucose
Establish relationships with patients to empower them in managing
their health
Required Qualifications
Current, valid, and in good standing Alaska Nursing License
(LPN/LVN)
3+ years of experience in cardiology, primary care, home care, or
chronic care management
Experience in remote monitoring and/or virtual care management
programs (preferred)
HIPAA-compliant area to work remotely and a distraction-free work
environment
Reliable internet connection with minimum 100mbps download and
10mbps upload speed
