Our flexible scheduling allows for one-on-one patient time.Benefits Registered Nurse Hospice Inpatient Unit MishawakaEmployer supplemented group health insurance which includes an annual on-site wellness screening; wellness and healthy lifestyle discounts; also includes $500 annual contribution to Health Savings AccountStudent Loan Forgiveness through PSLFWellness Benefit - Many routine testing, exams, and immunizations are covered 100%.Health Savings Account - CHC contributes up to $500 per year at the beginning of the year.Disability BenefitsFlexible Spending Account - Able to deduct up to $2,000 (pre-tax) each calendar year for reimbursement of uninsured medical, dental, or visions expensesHealthy lifestyle reimbursement program Reimbursement for items such as gym memberships, spin classes, yoga classes, etc.Generous retirement matchAfter 90 days of employment, CHC matches 25% of employee contributions up to a maximum annual match of $4,000; 100% vestingEmployer-paid life and AD&D insurance at 1x annual salaryEmployer-paid long-term disability insuranceEmployee Peer Recognition programAnnual anniversary/retention bonus - Upon completion of each year of employment, CHC will give $100 per year with a maximum of $1,000Accrued VacationJob Description Registered Nurse Hospice Inpatient Unit MishawakaPerforms an initial and ongoing comprehensive assessment of the patients and familys physical, psychosocial, environmental, safety and spiritual needs.Develops an individualized plan of care based on a comprehensive assessment in conjunction with the patients and familys goals.Prepares, administers, and supervises holistic patient care.Assesses all aspects of the patients pain and comfort, developing and implementing an individualized pain management plan, in collaboration with the hospice medical staff.Consults and collaborates with the interdisciplinary team (IDT) and other agencies involved in the patients care.Supports the patient, family, and caregiver in medical decision making and unique spiritual and cultural beliefs.Performs admissions as assigned.Communicates appropriate information and participates at IDT meeting, as needed.Demonstrates proper body mechanics when performing patient care, utilizing appropriate techniques to accomplish tasks.Accurately records and reports pertinent facts and observations regarding patients.Communicates appropriate information during shift change.Helps to provide continuity of care between team members by ensuring verbal and written reports are clear, concise, and timely.Documents any and all teaching in patient record.Teaches patient self-care.Teaches those parts of patient care appropriate for each family to provide.Assists the patients family with understanding the illness, plan of care, and needs of the patient.Assists the patients family, or ECF staff, with understanding the illness, the dying process, plan of care, and needs of the patient.Teaches the patient and family their rights regarding hospice care and benefits.Serves as preceptor/mentor to other staff nurses and Aides.Committed to Agency resolution against fraud and abuse.Knowledgeable in hospice philosophy and state and federal rules and regulations.Arrives to work and meetings on time and ready to work.Respects confidentiality of patients/families and other Agency employees.Attends and completes yearly nursing skills validations.Sees change as an opportunity and maintains a level of flexibility, which allows for adaptation to new ways of performing.May be required to work either IPU based on staffing needs and/or acuityAccountable to supervise/direct CNAsConditions of Employment for Registered Nurse Hospice Inpatient Unit MishawakaCompliance to Annual Flu Shot Policy and COVID Vaccine or ability to provide exemption documentationProvide an active auto insurance policy as a licensed driver in the State of Indiana with the proper level of coverage as directedProvide an active drivers licenseSuccess Capabilities of Registered Nurse Hospice Inpatient Unit Mishawaka and Elkhart:Licensed RN in State of IndianaExperience preferred in hospice care, home health, oncology and/or pain and symptom management.Current CPR certificationMust have and maintain a valid drivers license, good driving record and auto accident insuranceNOTE: The responsibilities of this position are described above, and they may be subject to change at any time due to reasonable accommodations or other reasons.
