People who will delicately guide patients through an unknown journey ahead; who serve the needs of family members with the same care as their patients; who will always be there to answer questions, so no one is left in the dark. As an Extended Care Registered Nurse (RN) at Emmanuel Hospice, you will serve alongside a dynamic team in a full-time (salaried) position. 7 days on (Monday through Sunday overnight and weekend) followed by 7 days offResponsibilities:Assisting the after hours team and additional meetings and holidays as required for the roleProviding holistic care to the dying and their loved ones in a 50 mile radius of Grand Rapids, MIAssessing the physical, psychological, social and spiritual needs of terminally-ill patients alongside our weekend/extended care teamAssisting the patient and family with the Plan of Care to meet their needs and monitor goals, while suggesting appropriate, comprehensive and proactive interventionsWorking with community Physicians and the Hospice Medical Director to ensure that each patient has individualized medical care including medications, equipment, and supplies that work towards meeting patient and family goalsWorking creatively in unison with other weekend nurses and an interdisciplinary team including the patient and their support system, in the planning, implementation, and evaluation of medical careEducating patients, families, caregivers, and community members about hospice services, and issues related to end of lifeProviding exemplary care and service to all those you encounter in your workQualifications:Current registered nurse (RN) license requiredHospice, home care, case management, or acute care experience requiredExperience with EMRs requiredBenefits: Sign-On Bonus Available!Competitive compensation up to $95,000 per yearMileage reimbursement of $.67 cents a mileCompetitive PTO package403(b) plan with matchGenerous Bereavement leaveMedical, Dental, and VisionEmployer paid Life and Short-term Disability based on employment statusSelf-care coachingEmployee Assistance ProgramTuition reimbursement and continuing education opportunitiesEquipment will be provided as needed: a computer and/or ipad or cell phone Emmanuel Hospice is committed to the work of anti-racism and pursuit of diversity, equity, and inclusion, in order to foster a safe and transformational environment for staff and patients to work, receive care, and thrive in their everyday lives.
