  • Practice Setting: Hospital-based

    • Trauma Level: Level 2
    • Required Clinical Areas: Non-Surgical and Surgical/Invasive Credentialing Information:
      • Estimated Timeframe: 3 months Requirements:
        • Board Certified Ophthalmologist Required
        • MI License Required
        • BLS/ACLS Not required unless performing moderate or deep sedation Facility Information:
