Job Description: This role plays a vital part in the healthcare team, delivering comprehensive care to specified patient populations and families. The position involves managing patient calls requiring medical triage and coordinating care during transitions after inpatient medical stays. The role also includes providing guidance, education, and facilitating resources for at-risk populations. This is a fully remote position, serving a region of the health system.

• Shift Details: Full time (40 budgeted hours), Monday to Friday. No weekends, no holidays.
• Additional Details: Inbound triage for primary care and post-hospital transitional care calls. Must live in Montana with easy access to one of our care sites near Butte, Bozeman, Billings, or Miles City.
• Preferred Qualifications: Acute care experience required, triage experience preferred. Essential Duties and Responsibilities:
• Performs clinical triage, including a comprehensive assessment, disposition, and education using established protocols
• Manages transitions of care to ensure appropriate follow-up care and mitigate risks associated with readmission
• Contacts patients following emergency department visits to ensure appropriate follow up
• Uses registry data to identify gaps in services and initiate intervention
• Refers patients to various resources, including behavioral health, transportation services, nutrition, etc.
• Educates patients and families about health status, health maintenance, and management of acute and chronic conditions
• Provides patient care based on practice guidelines, standards of care, and federal/state laws and regulations
• Documents patient assessment and intervention data using established medical record forms/automated systems and documentation practices
• Participates in multidisciplinary teams to improve patient care processes and outcomes
• Interacts with all staff, colleagues, and team members in a professional and collegial manner Minimum Qualifications: Required:
  • Graduate of an accredited school of nursing
  • Current/valid Registered Nurse license from the state licensing board is required. Basic Life Support (BLS) or Advanced Care Life Support (ACLS) is required.
  • One (1) year related experience in a clinic or acute hospital nursing setting is required Preferred:
    • Bachelor of Science in Nursing is preferred
      • Regular attendance to perform work on site during regularly scheduled business hours or scheduled shifts is required

Actual hourly rate dependent upon experience. $34.19 - $50.73
