Key Responsibilities
Support and optimize the Referral Coordination function to improve
referral yield
Manage and develop a Referral Coordination team to facilitate
referral processes
Conduct outreach to clients and partners to gather information for
care navigation
Required Qualifications
3-5 years of experience in Care Coordination/Referral
Coordination
2-3 years of experience leading Referral Management teams in
healthcare
Experience with end-to-end client conversion processes using
technology and human intervention
Familiarity with Salesforce and data analytics platforms
Experience working with Enterprise Partners to enhance access to
care
